Undetectable HIV: what U=U does and does not mean

U=U means that a person living with HIV who takes treatment and maintains an undetectable viral load does not transmit HIV through sex. The key conditions are treatment and sustained viral suppression. The statement is about sexual HIV transmission—not a declaration of a person’s current result, cure or protection from other infections.

Separate viral load from an HIV screening test

An undetectable viral-load result measures virus in someone receiving HIV care; it is not the same as a negative screening test in a person checking for infection. Ask the treating team how viral load is monitored and whether suppression has been maintained. Do not infer undetectability from feeling well, the time since diagnosis or a partner’s statement that one old result was low.

Keep the evidence tied to its route

CDC guidance states that people who get and keep an undetectable viral load do not transmit HIV to sexual partners. That evidence should be communicated clearly without stigma. It does not mean HIV has left the body, and it does not prevent gonorrhea, syphilis, herpes or other STIs. Do not extend the sexual-transmission statement into a universal zero-risk claim for shared injecting equipment or breastfeeding.

Ask the team when the conditions are uncertain

Missed treatment, interrupted care or a missing recent monitoring plan needs the treating team’s advice, not an assumption that yesterday’s status applies indefinitely. Discuss other STI prevention and pregnancy intentions separately. If a possible exposure has already happened and suppression is uncertain, seek prompt professional assessment rather than waiting for a routine booking or a future viral-load result. This guide cannot establish either partner’s current suppression status.

Common questions

Does undetectable mean HIV-negative?

No. Viral suppression on treatment is different from absence of HIV infection or a screening-test result.

Does U=U prevent every STI?

No. It concerns sexual HIV transmission under sustained suppression, not other infections.

Can an old result certify current undetectability?

No. Ongoing treatment and monitoring with the treating team establish whether suppression is maintained.
